Planning to Use Insurance?
Your insurance is responsible for processing your insurance benefits, letting you know your benefits when you ask for them, and letting you know your financial responsibility to the services we provide after they process your claim. We can only guesstimate what your financial responsibility will be beforehand because your insurance does not let us know your benefits in detail.
Please make sure to call the phone number on the back of your insurance card to understand your benefits.
When you call your insurance company, you may want to ask what your benefits are for these service codes when they are performed by chiropractors with tax-ID 47-3068481:

We can bill your insurance as a courtesy to you at no charge, but you also have the option of obtaining a Superbill from us to submit it yourself. We also provide receipts for HSA or FSA reimbursement upon request.
